Examples of the reasons for a visit include: sick or acute health needs (e.g. bronchitis, sinusitis), chronic health needs (diabetes, high blood pressure, asthma, physical exams, well and sick child exams or physicals), immunizations, labs/blood draws, in-office quick testing (e.g. strep testing), Pap tests, family planning, Hepatitis C treatment, and school physicals., Primary Care
We offer the following services: Immunizations/Vaccines, Infectious Disease Screening/Testing, Medication Assisted Treatment (MAT), Primary Care, Sick or acute health needs (e.g. bronchitis, sinusitis), chronic health needs (diabetes, high blood pressure, asthma, physical exams, well and sick child exams or physicals), Immunizations, labs/blood draws, in-office quick testing (e.g. strep testing), Pap tests, family planning, Hepatitis C treatment

The age populations we serve are: Adolescents, Adults, Children
Unfortunately we are not able to serve: Pregnant Women; Individuals with Certain Medical Conditions; Individuals who require specialty care, such as obstetrics, would be referred to a specialist. Other health conditions outside of the scope of primary care are referred to specialists, such as gastroenterology, neurology, oncology, etc.
The payment types we accept are: Commercial Insurance, HIP – Anthem, HIP – CareSource, HIP – MDWise, HIP – MHS, Medicaid, Medicare, Self pay, Sliding Scale Available (if uninsured)
